Observation

small colored rocks and other small items were seen in the toddler room straps outside wrapped around the arch were seen to be coming undone, resulting in longer pieces of the straps that could pose a risk to form a loop around a child's neck plastic bags were seen in the toddler room accessible to children the wood on the platforms outside were broken, wooden poles for the platforms were rotting, protruding nails were exposed on the wooden structures- posing a hazard of sharp edges and splinters the rubber mat at the bottom of the slide is lifting up, posing a tripping hazard the outlet in the toddler/staff bathroom is not Tamper Resistant, but is GFCI type the plastic bag and choking hazards have been removed.
